Subject[v3 10/12] x86/fsgsbase/64: Enable FSGSBASE by default and add a chicken bit

Now that FSGSBASE is fully supported, remove unsafe_fsgsbase, enable
FSGSBASE by default, and add nofsgsbase to disable it.
