Hello,
[1.] One line summary of the problem: GPF and 'attempt to access beyond end of device' errors
[2.] Full description of the problem/report: last night one of our servers had a protection fault followed, as you can see, by an 'attempt to access beyond end of device' error, which repeated for about 2hours until the server was rebooted. there were errors on the root partition which were corrected by e2fsck. server was up for about 10 days.
[3.] Keywords: kernel
[4.] Kernel version (from /proc/version): Linux version 2.2.10 (root@Cnn.CODEC.Ro.) (gcc version egcs-2.91.66 19990314/Linux (egcs-1.1.2 release)) #23 Tue Jun 22 17:24:34 EEST 1999
[5.] Output of Oops.. message (if applicable) with symbolic information resolved: (syslogd innterpreted the oops so I think this is good)

(the newline between the oops and ext2 error was inserted by me)
[6.] A small shell script or example program which triggers the problem (if possible) sorry.
[7.] Environment [7.1.] Software (add the output of the ver_linux script here) -- Versions installed: (if some fields are empty or looks -- unusual then possibly you have very old versions) Linux Spot.CODEC.Ro. 2.2.10 #23 Tue Jun 22 17:24:34 EEST 1999 i586 unknown Kernel modules 2.1.121 Gnu C egcs-2.91.66 Binutils 2.9.1.0.23 Linux C Library 2.1.1 Dynamic linker ldd (GNU libc) 2.1.1 Procps 2.0.2 Mount 2.9o Net-tools 1.52 Kbd [option...] Sh-utils 1.16 Modules Loaded spot ppp_deflate bsd_comp serial ppp slhc ne 8390 unix
spot is a module written by us but it worked without any of this errors with 2.0.36

[7.4.] SCSI information (from /proc/scsi/scsi) no SCSI.
[7.5.] Other information that might be relevant to the problem (please look in /proc and include all information that you think to be relevant): after the GPF there were 5 quiet seconds and after that is begun to print a lot of:
<6>attempt to access beyond end of device Jul 8 06:41:25 Spot kernel: 03:01: rw=0, want=124061557, limit=1229728 Jul 8 06:41:25 Spot kernel: EXT2-fs error (device ide0(3,1)): ext2_write_inode: unable to read inode block - inode=126514, block=124061556 Jul 8 06:41:25 Spot kernel: attempt to access beyond end of device Jul 8 06:41:25 Spot kernel: 03:01: rw=0, want=124061578, limit=1229728 Jul 8 06:41:25 Spot kernel: EXT2-fs error (device ide0(3,1)): ext2_write_inode: unable to read inode block - inode=34881, block=124061577
until 7am when was shutdown:
Jul 8 07:00:33 Spot kernel: attempt to access beyond end of device Jul 8 07:00:33 Spot kernel: 03:01: rw=0, want=124716915, limit=1229728 Jul 8 07:00:33 Spot kernel: EXT2-fs error (device ide0(3,1)): ext2_write_inode: unable to read inode block - inode=126514, block=124716914
after this there are a lot of '0's in the log.
the kernel is plain 2.2.10 with 'spot' module inserted.
hope this helps.
