Subjectsmp_invalidate_needed IPI wait (CPU#0)

I am finally getting closer to running 2.1 series on my SMP box.  The io_apic 
stuff is at least working enough to let me boot. Now I am getting a lot of the
following errors on bootup and running under any kind of load.

stuck on smp_invalidate_needed IPI wait (CPU#0)
stuck on smp_invalidate_needed IPI wait (CPU#0)

1. What does this mean?

2. How do I fix it?

Same error shows up in 2.1.90, 2.1.91-1, 2.1.91-2.

Here is a complete bootlog from a serial port hence some holes around the
serial initialization:

The file is a log from bootup to login prompt to give you an idea of how many
of these messages I get. More details may be obtained by request.
