Subjectpre-patch-2.1.45: kernel oops on pipe cleanup

I got the enclosed kernel oops while shutting down.  It looks like a
PIPE_WAIT structure was corrupted (the very first "next" pointer had
value 0x0002052b when "pipe_read_release" called
"wake_up_interruptible"), but I haven't been able to track down the
source of the problem.

The kernel was a pre-patch-2.1.45 with my O_CREAT patch, but I can't
see how that patch would have caused this error.
