The most important things you'll already find in Documentation/Changes. This is a good point to start. I have done the upgrade some days ago, and had to change only a few things (I'am running RedHat 4.1).
* if you want to use modules, you have to upgrade to modutils-2.1.34 (this is mandatory, older modutils won't work with 2.1).
* the net-tools (ifconfig, route, arp et.al) need to be upgraded to v 1.33. Imho, this isn't critical, but older version of ifconfig will present you wrong output.
* I also switched to libc-5.4.23 and binutils-2.8
The Changes file mentions some other packages, which should be upgraded, and http://www.linuxhq.com is another good source of information.
