pthread_mutex_t and pthread_cond_t were replaced with the new wrapped struct/functions. To avoid default initialization values, use of PTHREAD_MUTEX_INITIALIZER was replaced with mutex_init/destroy. This wasn't done for dso__data_open_lock due to no clear early initialization code.
