As per 7.4.3.1 the second index refer to Cb (0) and 1 Cr(). And the 32
is the maximum number of RefPicList0, as limited by the range of
num_ref_idx_l0_active_minus1 (if I read properly). This will never
change, so if we add defines it would only be for documentation purpose
I think.
