I can tell you what worked for me in this situation. It's a bit counter to what the HOWTO says, but it works for me! I've mailed the HOWTO maintainer, but got no answer.
I enabled IDE/ATAPI CDROM support (counter to what the HOWTO says). I did enable SCSI hostadapter emulation and generic SCSI CD-ROM support as suggested.
I added to my "append=" line in /etc/lilo.conf saying hdd=ide-scsi and hdc=cdrom. With my setup that allowed the SCSI emulator to only see the CD-RW drive and the regular IDE/ATAPI driver to see the CD-ROM drive.
I've attached my lilo.conf file and the configuration file I used to build the 2.2.1 kernel since the above may not be the clearest explanation in the world. I believe the only changes/additions to what the HOWTO says is the enabling of the IDE/ATAPI CDROM support and the additions to my append line.
