Check the kernel boot messages, specially the ones about the IDE. Maybe somehow the kernel is just not finding the card...
I had the same trouble with a SCSI disk and an AHA2749 (or something). The kernel was not able to find the card, thus could not mount the root FS and gave me an identical message, except it was 8:01 (from memory:).
If you use non standard addresses, you're gonna need to "append" some parameters using LILO.
